First Visit

On your first visit, Dr. Herzog, a Board Certified Physician specializing in non-surgical chiropractic orthopedics and pain management, will conduct a thorough history and physical examination.

All diagnostic studies and laboratory data are reviewed. Your physician will then formulate a treatment plan which best suits your individual needs. During subsequent visits, one or more modalities may be used to treat your painful condition.

At the time of your first visit you will need to bring with you the following:

A referral letter from your primary doctor describing your condition (if applicable).
Results of all lab tests, x-rays, CT scans, MRI’s and EMG’s which pertain to your condition.
All insurance cards and information.
